Please read the following requirements before submitting this form.

1. Successful Referral: The referred athlete must be a new participant in RVA's competitive Club program.

2. Full-Season Commitment: The referred athlete must complete the full Club season in good standing to qualify for a referral reward.

3. Separate Submissions: The referring family and referred family must each submit this form individually and select the appropriate role.

4. Referral Verification: Both families must provide accurate information so RVA can match and verify the referral.

5. One Referral Per Athlete: Each referred athlete may be credited to only one referring family. Duplicate or disputed referrals will be reviewed by RVA.

6. Reward Amount: The referring family will earn $100 per successful referral that meets all program requirements.

7. Reward Timing: Referral rewards are processed at the end of the Club season, after the final tournament and once all payment obligations have been fulfilled.

8. Initiating the Reward: The referring family is responsible for contacting the referred family to initiate the referral process. Once all eligibility requirements have been met, please email RVA. Rewards are not automatically applied.

9. Final Verification: Real Volleyball Academy will verify eligibility before approving a referral reward.
